What are the Requirements?
- Letter request for RO employees to travel abroad. Indorsement Letter from the Division Office (for SDO/School Level officials and employees) for authority to travel abroad.
- Duly accomplished CS Form 6 (RO/SDO/School officials and employees)
- Clearance from money and property accountabilities.
- Provident Clearance from RO/SDO/School, whichever is applicable.
- For School Heads (SHs): Designation Order from the Schools Division Superintendent (SDS) of an Officer-In-Charge to take care of the school while the SH is on travel.
- For Teachers: Certification from the Principal duly approved by the SDS of a substitute teacher to take the load of the teacher who is on travel abroad.
- For travels abroad on OB or OT, accomplished Travel Authority Request Form A (per DepED Order No. 43, s. 2014) and requests duly approved in the Regional Office should reached the Central Office at least 15 days before departure.

- For leave under Magna Carta For Women: Medical Certificate specifying that illness is related to the female reproductive organs and number of days to recuperate.

Under oath and written in a clear, simple, and concise language so as to inform the person complained of, about the nature and cause of the accusation against him.

3. A narration of the relevant and material facts which show the acts or omissions as allegedly committed by the person
